An Analysis of Nursing Research on Child Rearing in Korea.

  • Purpose
    The purpose of this study was to analyze papers on child rearing in Korea and to suggest future research and nursing practice directions for child rearing.
  • Methods
    An on-line search via 5 web sites yielded 201 research papers. The analysis of key concepts was based on the attributes of parenting and type of knowledge held by the parents.
  • Results
    The research papers found in the search were 27.3% experimental and 72.7% non-experimental. Among the participants, 77.0% of parents raising the child were mothers and the children were mostly young children under the age of one year. Types of knowledge related to child rearing varied widely, but explanatory knowledge (55.2%) was the most common, followed by prescriptive knowledge (30.0%) and descriptive knowledge (14.8%). For attributes of parenting, reaction to parenting (49.9%) followed by parenting behavior/practice (43.1%) was most common.
  • Conclusion
    Findings of this study indicate that further research is necessary regarding fathers and surrogate caregivers and on prescriptive knowledge for health promotion of children at different developmental stages. Also the results suggest a need for research toward developing child care policies.

  • 목적
    본 연구의 목적은 국내 간호학분야에서 수행된 양육에 대한 연구논문을 양육의 속성과 지식체 유형을 중심으로 분석하여 추후 자녀양육에 대한 연구와 간호실무의 방향을 설정하는 근거자료를 제시하는 것이다.
  • 방법
    KoreaMed, 한국학술정보, 과학기술정보통합서비스, 과학기술학회마을, 한국교욱학술정보원의 5개 온라인 사이트를 통해 201편의 논문을 선정하였고, 개념을 양육의 속성과 지식체 유형별(기술적, 설명적, 처방적 지식)로 분석하였다.
  • 결과
    연구설계는 실험연구가 27.3%, 비실험연구가 72.7%였고, 연구대상자는 양육자로는 어머니가 가장 많았고(36.0%), 피양육자로는 신생아가 가장 많았다(29.6%). 양육속성별로는 양육에 대한 반응 영역이 가장 많았고(49.9%), 양육지식 영역이 가장 적었다(2.2%). 양육속성의 하위영역별로는 양육에 대한 반응 영역에서는 양육스트레스, 양육자신감/효능감. 양육태도 등의 순이었고, 양육실제 영역에서는 애착, 부모-자녀 상호작용, 양육행동/양육실천 등의 순으로 나타났다. 아동의 성장/발달/행동 영역에서는 영아의 성장과 발달, 행동/건강 문제 등의 순으로 나타났다. 지식체 유형별로는 설명적 지식이 가장 많았고(55.2%), 처방적 지식(30.0%), 기술적 지식(14.9%)의 순으로 나타났다.
  • 결론
    자녀양육과 관련하여 다양한 지식체가 개발되었으나 처방적 지식의 개발이 미흡하며, 발달단계별 양육과 어머니 외의 다른 양육자 대상 연구가 부족하였고, 양육의 실제적인 측면보다는 양육에 대한 반응에 관한 연구가 더 많았다. 따라서 양육의 공동 책임자인 아버지와 대리 양육자 대상의 연구, 아동의 발달 단계별 건강을 위한 아동간호학의 차별화된 지식 개발 연구, 양육실제를 유도하는 처방적 지식을 개발하는 연구와 아동양육의 국가적 중요성을 감안하여 아동양육정책개발 연구를 제언한다.

연구의 필요성
아동의 성장발달과 건강의 유지증진은 출생 이전의 생물학적, 유전적 영향을 받기도 하지만 출생 후의 물리적, 사회적, 정서적 양육환경에 의해 크게 영향을 받기 때문에 양육의 질은 아동에게 매우 중요하다고 할 수 있다. 바람직한 양육의 결과는 부모와 자녀에게 성장과 만족을 가져오지만, 바람직하지 못한 양육의 경우 아동으로 하여금 정상적인 신체발달이 이루어지지 않게 하거나 정서적 결핍을 초래하기도 한다(Lee, 1998). 따뜻하고 지지적인 양육은 긍정적인 인지, 행동, 감정, 신체 결과와 관련되는 반면, 엄격하고 학대적이며 정서적으로 방임하는 양육은 아동기와 성인기에서의 신체적, 정서적, 정신적, 행동적 건강문제와 연관되며, 이러한 양육은 아동에게 나타나는 다양한 결과들의 20-50%를 설명한다(Waylen & Stewart-Brown, 2009).
양육이란 ‘아동을 한 인간으로 기르는 것’을 의미하며, 부모가 자녀를 보살피는 활동이고, 아동의 성장발달에 절대적인 영향을 끼치며, 불변의 형태가 아니어서 시간, 공간, 대상에 따라 변하면서 노력과 정성을 필요로 한다(Lee, 1998). 이러한 양육 혹은 양육에 대한 부모역할은 다양한 속성을 가지고 있어서 여러 학자들(Bigner, 1985; Kim, 2002; Kim & Han, 2009; Lee, 1992; Lee, 1998; Sim, Seo, & Kwon, 1998)이 제시한 속성들을 종합하면 애정결속, 신체적 돌보기, 훈육과 교육, 질병 및 사고 예방, 성장발달 지지, 부모역할 획득과 양육에 대한 반응 등 양육의 대상자로서의 아동뿐만 아니라 양육자 자체에 관한 다양한 측면이 포함됨을 알 수 있다. 양육의 이러한 다면적인 속성은 적절한 양육이 이루어지기 위해서는 아동을 위한 양육의 내용과 방법, 아동에게 나타나는 결과뿐만 아니라, 양육자의 양육능력과 양육에 따르는 반응 등에도 관심을 가져야 함을 시사한다.
양육자는 적절한 양육행동을 위한 지식과 기술뿐만 아니라 이를 실제에서 실천할 수 있는 능력에 대한 자신감이 필요하다(Bryanton, Beck, & Montepare, 2013). 그러나 많은 새로운 부모들이 적절한 양육행동을 배울 수 있는 사회적 지지나 역할모델을 가지고 있지 않다(Bryanton et al., 2013). 우리나라도 대가족 속에서 많은 수의 자녀를 부모세대와 함께 양육하면서 자연스럽게 양육법을 전수받았던 과거와는 달리 부부중심의 핵가족이 주를 이루는 현대 사회에서는 가족 내에 뚜렷한 역할모델이 없기 때문에 부모역할과 양육실제에 오류를 범할 수 있고, 부모 스스로도 불안과 스트레스를 경험할 수 있다(Kim, 2002). 따라서 부모를 포함한 아동의 양육자가 올바른 양육을 수행하고, 양육실제에 부모가 긍정적으로 적응할 수 있도록 지지할 필요가 있다. 국내외의 다양한 학문분야에서 아동양육에 대해 많은 연구가 수행되어 온 것은 이러한 필요성을 반영한 결과라 하겠다. 특히 간호사는 아동양육 관련 전문가 집단 중에서 의료기관 및 지역사회에서 초기 아동과 부모에게 접근하기가 가장 용이한 위치에 있고, 인간의 발달과정과 아동의 건강 및 돌보기에 대한 충분한 실무지식 및 기술을 가지고 있어서 부모역할을 개발시키고, 양육에 대한 정책을 제안할 수 있는 가장 적절한 전문직(Han, 2000)이기 때문에 아동양육에 대해 실무 및 학문적 관심을 기울여 왔고, 이에 양육에 대한 다양한 연구가 수행되어 오고 있다.
특정 학문분야에서 보다 좋은 연구방법을 구상하고 지식체의 영역을 확장하기 위해서는 새로운 방법론이나 지식체의 개발뿐만 아니라 선행 연구논문들을 분석하여 그동안 축적되어 온 탐구경향을 밝히고 이를 바탕으로 강점과 약점을 통찰하여 서로 공유함으로써 추후 연구와 지식체의 생성 방향을 조망해 볼 필요가 있다(Lee, Lee, Kim, Kim, & Park, 2004). 우리나라에서 현재까지 양육에 대한 많은 연구들이 수행되어 왔으나 간호학문에서 이들을 분석한 연구는 모아관계(Paik, Kim, Wang, Sung, & Cho, 1996), 모아상호작용(Oh & Lee, 2003), 부모-자녀 관계(Kim, Park, & Park, 2010) 등과 같이 초기 아동기의 양육자-자녀의 관계라는 특정 영역에 관한 논문의 분석에 집중되어 있고, 양육의 포괄적 영역에 관한 논문을 분석한 연구는 찾아보기 어려웠다.
Kim 등(2004)은 간호학에서 연구개념을 체계적으로 분석하기 위해 다양한 분석틀이 이용되고 있으나, 이들은 간호학의 전반적인 탐구경향이나 지식체 생성에 관한 새로운 조망을 탐색하는 데 미흡하다고 지적하면서, 연구개념을 철학, 지식체 유형, 방법론으로 분석하는 틀을 제안하였다. 특히 지식체 유형을 분석하는 분석틀은 Dickoff와 James (1968)가 제시한 이론의 수준과 유사하여 특정 분야에서 개발된 지식체의 수준을 분석하는 데 유용하게 활용할 수 있다(Kwon, Seo, & Kim, 2012).이에 본 연구는 국내 간호학분야에서 수행된 가족 양육자의 자녀양육에 대한 논문을 Kim 등(2004)의 지식체 유형을 분석하는 분석틀을 이용하여 아동양육에 대한 지식체 개발 현황을 분석하고, 추후 아동양육에 대한 연구와 간호실무의 방향을 설정하는 데 근거자료를 제시하기 위해 시도되었다.
연구 목적
본 연구의 목적은 현재까지 국내 간호학 분야에서 수행된 가족 양육자의 자녀양육에 대한 연구논문을 분석하여 아동양육에 대한 지식체 개발의 현황을 파악하고 추후 아동양육에 대한 연구와 간호실무의 방향을 설정하기 위한 근거자료를 제시하는 데 있다.
연구 설계
본 연구는 현재까지 국내 간호학회지의 게재 논문과 간호학 분야 학위논문 중 가족양육자의 자녀양육에 대한 연구논문을 분석한 서술적 조사연구이다.
연구 대상 논문
본 연구의 대상 논문은 국내의 간호학 관련 학회지와 간호학 전공 학위논문 중 가족 양육자의 자녀양육에 대해 수행된 논문이었다. 학회지별 대상 논문은 아동간호학회지 50편, 대한간호학회지 25편, 부모-자녀건강학회지 24편, 여성건강간호학회지 13편, 정신간호학회지 7편, 간호행정학회지 2편, 지역사회간호학회지 2편, 임상간호연구 2편, 재활간호학회지 1편, 한국보건간호학회지 1편으로 총 127편이었고, 학위논문은 74편으로 대상논문은 총 201편이었다. 학위논문은 간호학 전공 학위논문만을 모집단으로 하였고, 학위논문과 학회지 논문이 중복된 경우 학회지 논문을 선택하였다.
자료 수집 방법
대상 논문은 컴퓨터를 통한 온라인 검색으로 수집하였다. 논문의 검색은 National Evidence Based Healthcare Collaborating Agency (NECA, 2011)에서 제시한 우리나라의 5개 핵심 검색 데이터베이스 중 간호학 논문이 적은 한국의학논문데이터베이스를 제외하고, KoreaMed, 한국학술정보, 과학기술정보통합서비스, 과학기술학회마을의 4개 사이트와 학위논문 검색에 유용한(NECA, 2011) 한국교육학술정보원을 추가하여 5개 사이트에서 키워드로 2013년까지 등록된 논문을 검색하여 수집하였다. 검색어는 ‘양육’, ‘육아’, ‘상호작용’, ‘애착’, ‘부모역할’, ‘부모-자녀 관계’, ‘돌봄’, ‘영양관리’, ‘훈육’, ‘질병예방’, ‘사고예방’, ‘건강증진’, ‘성장발달’이었다. 검색어를 통해 해당 논문이 게재된 학회지 수는 10개였고, 선정된 논문의 수는 학회지 263편, 학위논문은 191편으로 일차적으로 수집된 논문은 총 454편이었다. 학위논문과 학회지 논문이 중복되는 경우 학회지 논문을 선택하였다. 일차적으로 선택된 논문 중에서 연구대상이 양육자로는 아버지나 어머니 혹은 조부모이고, 피양육자로는 건강한 아동, 치료과정이 끝나 퇴원을 앞두고 있거나 퇴원 후인 고위험신생아와 미숙아인 논문을 최종 선정하였다.
분석내용과 분석절차
선정된 논문은 발표연도와 연구대상자는 출현빈도별로 실수와 백분율을 산출하였고, 연구개념은 주개념을 중심으로 양육의 속성과 생성된 지식체의 유형 측면에서 분석하였다. 양육의 속성은 선행연구(Bigner, 1985; Kim, 2002; Kim & Han, 2009; Lee, 1992; Lee, 1998; Sim et al., 1998)에서 제시한 양육의 속성을 근거로 양육에 대한 양육자의 반응, 구체적인 양육의 내용과 방법을 포함하는 양육실제, 아동에게 나타나는 양육결과를 포함하는 아동의 성장발달과 행동, 양육지식 등의 4개 하부영역으로 분류하여 각 영역별로 출현빈도와 백분율을 산출하였다. 지식체 유형을 분석하는 분석틀은 Kim 등(2004)이 제시한 학문의 지식체 수준을 확인할 수 있는 지식체 유형 분석틀을 이용하였다. 여기에는 기술적(descriptive), 설명적(explanatory) 및 처방적(prescriptive) 지식이 포함되며, 기술적 지식은 현상의 정의와 현상의 의미를 찾아내어 현상이 어떤 형태로 일어나고 있는지 현상을 이해하는 지식이며, 설명적 지식은 어떤 현상이 어떻게 해서 일어나고, 왜 그렇게 일어나고 있는가를 설명하는 지식이고, 처방적 지식은 대상자가 가지는 문제, 결핍에 대응하여 정규적인 가치의 기준을 가지고 어떻게 바꾸는 것이 좋은지에 대한 목표를 가지고 있는 변화지향적 지식을 의미한다(Kim et al., 2004). 연구의 분류는 기술적 지식의 유형에는 서술적 조사연구, 질적 연구 등을 포함하고, 설명적 지식의 유형에는 상관관계연구, 구조모형개발연구, 도구개발연구 등을 포함하며, 처방적 지식의 유형에는 실험연구를 포함한다. 지식체 유형은 대상 논문의 연구설계를 중심으로 출현빈도와 백분율을 산출하였다.
분석절차는 연구자들이 먼저 논문을 분석할 수 있는 분석표를 작성한 다음 논문을 읽으면서 해당내용을 분석표에 기록하여 빈도를 합산하고 백분율을 산출하는 방식으로 이루어졌다. 분석은 한 연구자가 일차적으로 분석한 결과를 다른 연구자가 확인하였고, 상호 이견이 있는 경우 합의하여 결정하는 절차를 거쳤다.
연도별, 연구설계별 분포
연도별 분포는 1973-1989년까지 10편, 1990-1999년까지 26편, 2000-2013년까지 165편으로 나타났다(Table 1). 연구설계는 실험연구가 55편(27.3%), 비실험연구가 146편(72.7%)으로 나타났고, 비실험연구는 서술적 조사연구가 78편(38.8%)으로 가장 많았으며, 상관관계연구 29편(14.4%), 질적 연구 17편(18.5%), 문헌고찰 8편(4.0%), 방법론적 연구 5편(2.5%), 구조모형개발연구 4편(2.0%), 개념분석연구 3편(1.5%), 혼종연구 2편(1.0%)의 순이었다.

연구 대상자
연구대상자는 양육자와 피양육자(아동)로 구분하여 분석하였다(Table 2). 양육자는 어머니가 71편(36.0%)으로 가장 많았고, 그 다음이 초산모 35편(17.8%), 부모 15편(7.6%), 결혼이민자 어머니, 간호사, 아버지 각 13편(6.6%), 조모(외조모) 9편(4.6%), 저소득 양육자 7편(3.6%), 조기출산 산모 6편(3.0%), 취업모 5편(2.5%), 탈북여성 3편(1.5%), 이혼여성, 미혼모, 시각장애인, 주양육자가 각 1편(0.5%)의 순으로 나타났다.

피양육자(아동)는 신생아가 56편(29.6%)으로 가장 많았고, 영아 48편(25.4%), 발달단계 미구분 18편(9.5%), 취학 전 아동 17편(9.0%), 영유아 15편(7.9%), 학령전기 아동 11편(5.8%), 학령기 아동 9편(4.8%), 유아와 저출생 체중아/미숙아 각 6편(3.2%), 청소년 3편(1.6%)의 순이었다.
연구 개념
양육속성의 하위영역별 분포는 양육에 대한 반응이 178편(49.9%)으로 가장 많았고, 양육 실제 154편(43.2%), 아동의 성장/발달/행동 17편(4.8%), 양육지식 8편(2.2%)의 순으로 나타났다(Table 3).

연구개념을 분석한 결과 총 수는 357개로 나타났다. 양육속성의 하위 영역별로 보면, 양육에 대한 반응 영역에서는 양육스트레스/긴장/부담감이 54편으로 가장 많았고, 양육자신감/효능감 48편, 양육태도 18편, 부모역할 만족 16편, 정신건강(어머니 우울/심리적 안녕) 11편, 모성정체감(자아존중감) 8편의 순으로 나타났다. 양육실제 영역에서는 애착이 39편으로 가장 많았고, 부모-자녀 상호작용 33편, 양육행동/양육실천 27편, 양육경험 16편, 아동을 위한 건강관리/증진 14편, 양육환경 8편, 양육방법/유형/능력/속성 6편, 아동학대 5편의 순으로 나타났다. 아동의 성장/발달/행동 영역에서는 영아의 성장/발달이 8편으로 가장 많았고, 행동/건강 문제 5편, 가정 내 사고예방 2편, 아동의 상태 2편의 순으로 나타났다. 양육지식 영역에서는 양육지식이 8편으로 나타났다.
연구개념을 지식체 유형별로 분석한 결과 설명적 지식이 197편(55.2%) 으로 가장 많았고, 처방적 지식이 107편(30.0%)으로 그 다음이었고, 기술적 지식이 53편(14.8%)으로 가장 적었다.
양육속성의 하위영역별 지식체 유형은 양육에 대한 반응은 설명적 지식 119편, 처방적 지식 43편, 기술적 지식 16편의 순이었고, 양육 실제는 설명적 지식 72편, 처방적 지식 48편, 기술적 지식 34편의 순이었다. 아동의 성장/발달/행동은 처방적 지식 10편, 설명적 지식 6편, 기술적 지식 1편의 순이었고, 양육지식은 처방적 지식 6편, 기술적 지식 2편의 순이었다.
본 연구는 자녀양육에 대한 연구가 국내 간호학 학술지에 소개되기 시작한 1973년부터 2013년까지 간호학 학술지 게재논문과 학위논문 중 가족양육자의 자녀양육에 대한 연구논문을 양육속성과 지식체 유형을 중심으로 분석하여 추후 아동양육에 대한 연구와 간호실무의 방향을 제시하는 데 기초 자료를 제공하고자 시도되었다.
총 201편의 논문을 발표연도별로 분석한 결과 1989년 이전에 10편, 1990년대에 26편으로 2000년 이전에는 매우 적었으나, 2000년 이후 165편으로 급속하게 증가한 것을 볼 수 있다. 양육관련 연구의 증가는 학회지의 수 및 기존 학회지의 발간횟수 증가, 간호학자 및 대학원생 수의 증가에 따른 영향도 있을 것으로 생각되나, 보다 근본적으로는 사회경제적 수준이 상승되고 최근 저출산으로 인한 소자녀 경향에 따라 자녀양육에 대한 관심이 많아지고 있기 때문으로 보인다(Kim et al., 2010). 간호학에서 자녀양육에 대한 연구에 관심이 증가한 것은 아동발달의 주요 열쇠가 긍정적 양육이라는 사실(Bryanton et al., 2013)과 자녀양육 지지가 아동간호학의 주요 주제임을 고려하면 바람직한 현상으로 생각된다.
연구설계별로는 문제에 대한 개선적 변화를 지향하는 실험연구가 27.3%로 나타났는데, 아동간호학회지 게재 논문을 분석한 선행연구에서 실험연구의 비율이 1995년부터 2000년까지(Kim et al., 2002) 17%였던 것보다는 높았으나, 2005년부터 2009년까지(Kim et al., 2011) 27.7%와는 비슷하였다. 그러나 2007년부터 2008년까지 대한간호학회지 논문을 분석한 Shin 등(2010)의 32.2%와 1982년부터 2010년까지 한국 간호학 박사학위논문을 분석한 Lee 등(2012)의 51.5%보다는 훨씬 적은 것으로, 과학적 방법에 따라 수행된 중재는 양육의 효과성에 대한 근거로 축적되고, 교육적 중재가 자녀의 적절한 건강과 돌봄, 부모-자녀 관계를 증진시킨다는 결과(Bryanton et al., 2013)를 볼 때, 또한 중재를 개발하는 실험연구는 실무학문인 간호학에서는 필수적인 과제임을 고려하면 아동양육 분야에서 실험연구가 더 활발히 이루어져야 할 것으로 생각된다. 본 연구에서 연구개념의 지식체 유형을 분석한 결과에서도 처방적 지식이 30.0%로 나타나 추후 실험연구가 보다 활발하게 이루어져야 함을 알 수 있다.
연구대상자로서 양육자는 아버지 13편과 할머니 9편, 총 22편을 제외하면 모두 어머니 단독 혹은 어머니를 포함한 연구로 나타났는데, 이는 간호학에서 부모-자녀 관계 연구의 동향을 분석한 Kim 등(2010)의 연구에서 대상자가 어머니가 가장 많았다는 결과와 일치한다. 최근 여성의 사회활동 증가와 출생률의 저하, 이혼률의 상승 및 주말 가족이나 남성 한 부모 가족의 증가와 같은 가족 인구학적 변화에 대처하기 위해서뿐만 아니라, 아버지는 어머니와 상당히 다른 고유한 역할로 자녀양육에 기여하며, 아동의 발달은 아버지와 어머니로부터 각기 다른 자극을 받을 때 더 효과적이기 때문에 자녀양육에 있어서 아버지 도 어머니 못지않게 중요하다(Fagan & Palm, 2004)는 점에서 아동간호학에서 아버지의 양육에 대한 연구가 더 활발하게 이루어져야 할 것으로 생각된다. 또한 전국 보육실태조사(Korea Institute of Child Care and Education, 2012) 결과 부모가 맞벌이 하는 경우 0세 자녀의 87.6%는 (외)조부모가 양육하고 있고, Won, Lee, Kim, Yoo와 Park (2012)의 연구에 의하면 외조모의 손자녀 양육기간이 6개월에서 길게는 10년, 양육시간은 하루 3.5-12시간, 일주일 평균 4일로 나타나 조부모의 양육역할 비중이 크며, 손자녀의 부상이나 안전사고에 대해 두려움과 죄책감을 지니고 있다는 결과를 볼 때 조부모가 손자녀를 양육하면서 경험하는 양육반응과, 양육기술과 지식과 관련된 양육실제에 대한 연구가 필요한 것으로 생각된다.
연구대상자로서 피양육자인 아동은 퇴원을 앞둔 미숙아와 고위험신생아를 포함하면 신생아가 약 32.8%, 영아가 25.4%로 영아기까지의 어린 아동이 많은 비중을 차지한 반면, 학령기 아동 9편, 청소년은 3편으로 매우 적었다. 초기 발달 단계에 있는 아동의 양육자를 대상으로 많은 연구가 수행된 것은 간호학문이 신생아와 같은 어린 아동에게 접근이 용이하고, 어린 아동일수록 성장속도가 빠르며 건강문제가 발생할 가능성이 높기 때문에 부모의 세심한 돌보기가 필요하며, 초산모의 경우 양육경험이 없으므로 양육에 필요한 지식과 기술, 태도 등에 관한 중재가 요구되기 때문으로 생각된다. 그러나 아동이 성장 발달함에 따라 성장발달의 문제뿐만 아니라 건강문제도 달라지기 때문에 아동간호학이 다루고 있는 학문적 특성을 볼 때 아동의 성장발달과 건강증진을 위해 다양한 발달단계에 있는 아동의 양육자를 대상으로 하는 연구가 보다 활발하게 이루어져야 할 것으로 생각된다. 특히 청소년 건강위험행태조사 결과에 의하면 흡연율 13.3%, 음주율 27.8%, 비의료적 약물 경험률 6.8%, 자살사고율 23.7%, 자살시도율 5.8%로 청소년들의 건강행태에 문제가 있음(Korea Centers for Disease Control and Prevention, 2008)을 고려할 때 청소년의 건강위험행태를 줄이기 위해 양육자의 적절한 양육실천이 중요하고, 이를 지지할 수 있는 중재연구가 활성화되어야 할 것으로 본다.
연구개념을 양육속성의 하부영역별로 분석한 결과 양육에 대한 반응이 178편(49.9%)으로 거의 반 수에 해당하였다. 실천적인 양육행동이나 방법에 관한 연구보다 양육자의 양육반응에 관한 연구가 더 많은 것은 양육에 대한 반응이 자녀 양육과정에서 직접 경험하는 반응이기 때문에 자녀양육의 질과 양에 영향을 미친다는 점에서 중요한 측면(Kwon, 2013)임을 반영한 결과로 생각된다. 특히 양육반응 중 양육의 어려움을 나타내는 양육스트레스/긴장/부담감이 54편으로 가장 많았는데, 이는 부모-자녀 관계에서 부모 대상 주요 개념으로 ‘스트레스’가 가장 많았다는 결과(Kim et al., 2010)와 일치한다. 이러한 결과를 볼 때 과도한 양육스트레스는 부모로서의 능력에 회의를 느끼고 위축된 행동을 보이며, 체력부족, 불안, 우울, 사회적 고립감 등의 부정적 정서반응을 유발하여 자녀에게까지 부정적 영향을 미치기 때문(Kim & Park, 2009)에 양육스트레스는 자녀 양육에서 중요하게 다루어지고 있는 반응임을 알 수 있다. 따라서 양육자가 양육과정에서 경험하는 스트레스를 감소시킬 수 있는 간호중재를 개발하는 실험연구가 필요할 것으로 생각된다. 그러나 본 연구결과 양육스트레스와 관련되는 요인을 확인하는 설명적 지식을 생성하는 연구는 43편에 연구된 요인이 23개로 매우 다양한 데 비해 양육스트레스를 감소시킬 수 있는 중재개발을 위한 처방적 지식을 생성한 연구는 6편, 중재의 내용 또한 교육중재와 감각적 자극 및 접촉의 두 종류에 불과하였다. 따라서 보다 적절한 양육이 이루어질 수 있도록 양육스트레스와 관련되는 요인을 중심으로 스트레스/긴장감/부담감 감소를 위한 중재프로그램의 개발이 필요할 것으로 생각된다. 양육에 대한 반응 영역에서 양육자신감/양육효능감은 48편을 차지하였는데, 양육자신감과 양육효능감은 양육능력에 대한 만족감이나 신념으로써 바람직한 양육의 선행요인일 뿐 아니라 양육효능감이 낮은 양육자에게 양육을 받은 아동은 부적절한 행동을 나타내며 긍정적 발달에 방해를 받으므로 양육자의 양육자신감과 양육효능감은 양육자-아동의 관계에서 매우 중요하다(Oh, 2007)는 점을 반영한 결과로 생각된다.
양육실제 영역에서는 애착과 부모-자녀 상호작용 연구가 총 69편으로 전체 154편 중 가장 높은 비중을 차지하였다. 이러한 결과는 애착과 부모-자녀 상호작용이 부모-자녀 관계를 설명하는 대표적인 개념으로, 아동의 발달과 부모-자녀 관계의 질에 영향을 미쳐 자녀의 최적의 발달을 기대할 수 있게 하는 중요한 개념(Kim et al., 2010)이기 때문에 특히 초기 아동기에 중요하게 다루어지고 있는 연구 주제임을 알 수 있다. 지식체 유형별로 분석한 결과 설명적 지식에서 이들 개념과 관련되는 요인이 매우 다양함을 알 수 있고, 이러한 결과를 볼 때 애착과 부모-자녀 상호작용을 증진시킬 수 있는 다양한 중재방법을 개발할 가능성을 시사한다고 할 수 있다. 그러나 처방적 지식에서 중재방법은 감각적 자극 및 접촉과 부모교육에 집중되어 있어서 다양한 중재방법을 개발할 필요성이 있음을 보여준다. 양육실제 영역에서 양육의 실천적인 개념인 양육행동/양육실천에 관한 연구는 25편으로 높은 비중을 차지하였으나 양육자의 양육행동의 적절성을 증진시키고자 수행된 처방적 지식을 생성한 연구는 7편뿐이었고, 아동의 건강유지와 증진 또한 처방적 지식은 단 2편으로 매우 미흡한 것으로 나타났다. 아동의 건강습관이나 건강생활양식은 일생 동안의 건강에 결정적인 영향을 미치므로 양육자는 양육실천을 통해 아동이 건강한 습관을 형성하고 선택하도록 안내하고 지지해야 하는 역할을 해야 한다(Kim et al., 2009)는 점에서 일상적인 건강행동과 감염예방, 비만예방, 사고예방, 성폭력을 포함한 성문제 등 특정 건강문제를 예방하기 위한 건강행동을 습득하고 습관화시키기 위한 양육내용과 방법을 개발하는 데 관심을 가져야 할 것으로 생각된다. 아동간호학은 아동의 성장발달과 건강의 유지, 증진을 위한 돌보기에 대한 실무지식과 기술을 다루기 때문에 이러한 분야에 대해 다른 어떤 학문분야보다 심도 있는 연구를 수행할 수 있으며, 이러한 연구성과를 통해 아동양육과 관련된 다른 학문분야와는 차별화된 지식을 개발할 수 있을 것으로 본다.
본 연구결과 처방적 지식을 생성하는 연구에서 개발한 중재는 대부분 교육으로 나타났다. 이러한 결과는 입원아동 보호자를 대상으로 한 논문을 분석한 Kwon 등(2012)의 연구에서 중재의 유형이 대부분 정보제공과 교육으로 나타난 결과와 2007년부터 2008년까지 대한간호학회지 논문을 분석한 Shin 등(2010)의 연구에서 가장 많은 중재가 교육프로그램이었다는 결과와 유사하다. 이는 간호사의 중요한 역할 중 하나가 교육이나 정보제공을 통해 대상자가 스스로 건강을 관리할 수 있도록 준비시키는 역할(Kwon et al., 2012)임을 반영하는 결과로 볼 수 있다. 이러한 결과를 볼 때 자녀 양육에서도 양육자를 대상으로 하는 간호사의 교육이 중요한 중재의 수단임을 알 수 있고, 따라서 교육중재를 개발하는 경우 양육자와 피양육자의 특성을 고려한 다양한 방법과 내용을 포함하여야 할 것이다. 또한 양육관련 간호중재가 교육에만 국한되지 않고 양육의 실제적 측면을 고려하여 양육행동 수행에 관한 지식이나 실천의 정확성을 증진시킬 수 있는 중재프로그램이 개발되어야 할 필요가 있다고 본다.
본 연구는 간호학 학술지 게재논문과 간호학 분야 학위논문 중 가족 양육자의 자녀양육에 대한 연구논문을 양육의 속성과 지식체 유형을 중심으로 분석하여 추후 아동양육에 대한 연구와 간호실무의 방향을 제시하는 데 기초 자료를 제공하고자 시도되었다.
연구결과 자녀양육 관련 연구의 대상자는 어린 아동의 어머니에 편중되어 있어서 다양한 발달단계에 있는 아동의 다양한 양육자에 대한 연구가 미흡함을 알 수 있었다. 자녀양육과 관련하여 다양한 지식체가 개발되었으나 개발된 지식의 수준은 설명적 지식이 과반수를 차지하여 처방적 지식의 개발이 미흡하고, 처방적 지식의 중재방법 또한 교육에 집중되어 있는 것을 알 수 있었다. 양육의 속성별로는 양육의 실제적인 측면보다 양육자의 양육에 대한 반응에 관한 연구가 더 많은 비중을 차지하고 있음을 알 수 있다. 양육 속성 중 양육의 실제영역에는 애착과 모아 상호작용이, 양육에 대한 반응영역에는 양육스트레스, 양육자신감과 양육효능감, 양육태도 등에 대한 연구가 많이 이루어졌다. 이상의 연구를 토대로 자녀양육과 관련된 다음과 같은 추후 연구를 제언한다. 첫째, 양육공동 책임자로서 아버지 역할의 중요성을 반영한 연구와 대리 양육자로서 조부모, 또는 다양한 대리 양육자 대상의 연구를 제언한다. 둘째, 아동의 발달단계별 건강의 유지, 증진을 위한 양육내용에 관한 연구를 통해 아동양육 관련 다양한 학문분야에서 아동간호학의 차별화된 양육지식을 개발할 필요가 있다. 셋째, 적절한 양육실제를 유도하는 처방적 지식을 개발하는 다양한 실험연구를 제언한다. 넷째, 아동양육이 미래의 건강한 국민을 양성하는 매우 중요한 국가적 과제라는 점과 간호학이 이를 효율적으로 지지할 수 있는 위치에 있는 학문이라는 점을 감안할 때 아동양육정책을 개발하는 연구를 제언한다.
