Korean nurses’ knowledge, opinions and current practice of trauma-informed pediatric nursing care in South Korea: a descriptive study

Purpose
This study aimed to investigate the knowledge, opinions, competence, and barriers of nurses providing trauma-informed care (TIC) to children in hospitals.
Methods
Data were collected from 198 nurses nationwide using the TIC Provider Survey developed by the Center for Pediatric Traumatic Stress in July, 2024. Descriptive statistics, t tests, and one-way ANOVA were conducted using the IBM SPSS Statistics software (ver. 23.0, IBM Corp.).
Results
Of the 13 items assessing knowledge of TIC, seven had a correct response rate of over 90%. However, the correct response rate for these three items were less than 30%. Regarding opinions on the TIC, more than 90% of participants agreed with five of the seven items. In self-rated competence in providing TIC, they were not competent in responding to a child’s (or parent’s) questions about whether the child is going to die or understanding how traumatic stress may present differently according to developmental stages. The major barriers to providing TIC were time constraints, lack of training, and organizational support. Items with relatively low performance rates were ‘teach parents what to say to their child after a difficult/sick/scary experience, and teach child or parents the specific ways to cope with unpleasant experiences’.
Conclusion
This study suggests that it can contribute to the development of specific guidelines for nurses to effectively apply TIC in pediatric and family nursing practice, emphasizing the need for educational programs to support this implementation.

Unveiling the dynamics of stunting: a qualitative exploration of parenting patterns and toddlers aged 6–59 months in Bejiharjo, Indonesia

Purpose
This research was conducted to explore the experience of mothers in raising stunted toddlers aged between 6–59 months. It also examined the perceptions of parenting styles shaped by socio-cultural contexts and time constraints affecting the nutritional status as well as growth in Bejiharjo Village, Java, Indonesia.
Methods
A qualitative method was used and data were collected through in-depth interviews, diary recording, and focus group discussions with mothers of toddlers, stakeholders, and healthcare professionals. Transcript data from recorded interviews were processed using qualitative content analysis.
Results
The results showed that there were four emergent themes, namely “Toddler parenting patterns,” “Family and environmental health,” “Eating patterns and consumption habits of toddlers,” as well as “Literacy and understanding of parenting and child health.” Furthermore, eight related theme clusters were yielded from the analysis of parenting experiences. Differences between stunted and non-stunted toddlers in parenting patterns and daily activities were recorded. Interactions between toddlers and mothers as well as productive activities had a shorter average duration.
Conclusion
Mothers experience various challenges and limitations in raising toddlers with the risk of stunting. Therefore, increasing understanding and knowledge about the importance of healthy parenting and good nutrition are necessary to prevent stunting.

Purpose
The purpose of this study was to investigate perceptions of fatherhood among fathers of high-risk neonates and their confidence regarding neonatal care.
Methods
A study was conducted of 100 fathers whose neonates had been admitted to a neonatal intensive care unit (NICU) for at least 48 hours. Perceptions of fatherhood, neonatal characteristics, and fathers' confidence regarding neonatal care were measured using a self-reported questionnaire. The survey took approximately 10 minutes to complete and was completed anonymously. The factors that affected fathers' confidence regarding neonatal care were analyzed using multiple regression.
Results
The scores for positive and negative perceptions of fatherhood were relatively high at 4.27 and 3.42 out of 5 points, respectively. A positive perception of fatherhood was found to influence fathers' confidence regarding neonatal care.
Conclusion
NICU nurses should encourage fathers to effectively perform their parenting roles by implementing educational programs and interventions to promote positive perceptions of fatherhood and fathers' confidence regarding neonatal care after discharge.

Purpose
Safe sleep practices (SSP) are among the main strategies to reduce sleep-related sudden unexplained infant death (SUID). Daycare personnel must be knowledgeable and trained in SSP related to SUID. This study explored the experience, knowledge, and confidence regarding SSP associated with SUID of daycare personnel.
Methods
A cross-sectional survey was conducted with 395 staff members at 61 daycare centers to measure their experience related to SSP (10 items), related to sleep position and location, bedding materials, and other topics; knowledge of SSP (18 items); and confidence in SSP (1 item) related to SUID.
Results
A substantial proportion (23.6%) of respondents used the lateral or prone positions for infant sleep. On average, 4.5 bedding materials were used for infant sleep. Participants showed a lack of knowledge about SSP as indicated by a 56.6% knowledge of SSP related to SUID correct answer rate. Personnel who received SUID education were more knowledgeable and had more confidence regarding SSP than those who did not. More knowledge and confidence related to SSP were associated with better adherence to SSP.
Conclusion
Standard SSP guidelines should be developed based on South Korea's culture of childcare for educating both childcare professionals and parents at home.

Knowledge on sudden unexplained infant death-related safe sleep practices and infant cardiopulmonary resuscitation in pediatric nurses

Purpose
Sudden unexplained infant death (SUID) is a major contributor to infant mortality, and pediatric nurses have the responsibility to educate parents on SUID-reducing strategies. This study was conducted to measure pediatric nurses' knowledge of SUID-related safe sleep practices (K-SSSP) and infant cardiopulmonary resuscitation (K-ICPR).
Methods
In total, 136 pediatric nurses were administered a survey including K-SSSP (13 items), K-ICPR (5 items), confidence in K-SSSP education (1 item; 5 points), and other factors relating to SUID experiences or education.
Results
The correct answer rates of the K-SSSP and K-ICPR were 62.6% and 62.5%, retrospectively. The mean score for confidence in K-SSSP education was 2.6±0.9. Only 18 nurses (13.2%) responded that they educated parents on the content of the K-SSSP, while 76 nurses had received education on SUID. Positive relationships were observed between K-SSSP scores and higher education, between K-ICPR scores and having own child(ren) and clinical experience, and between confidence in K-SSSP education and higher education or having one's own child(ren). Nurses caring for newborns performed more SUID education than nurses working in other units.
Conclusion
There is a profound need to implement a systemic educational program on SUID and strategies to reduce SUID for pediatric nurses.

An Analysis of Pediatric Emergency Nursing Practice and Nursing Competence among Emergency Department Nurses

Purpose
The purpose of this study was to examine the frequency, perceived importance and competence of pediatric emergency nursing practice (PENP) in nurses who cared for pediatric patients in the emergency department.
Methods
This cross-sectional descriptive study analyzed 175 emergency department nurses caring for pediatric patients at 7 university hospitals with more than 500 beds, located in Seoul and Gyeonggi Province. The measurement tool was modified from the Classification of Standard Nursing Activities to measure the frequency, importance of PENP, and nursing competence. It comprised 143 items in 16 domains of PENP.
Results
The most frequent nursing practice was the domain of ‘nursing records and environmental management’ and the least frequent practice was the ‘research and consulting’ domain. The nursing care domain perceived to be most important by nurses was ‘specialized intensive nursing care’. The nursing care domain with the highest level of nursing competence was ‘hygiene care’, and the domain with the least level of nursing competence was the ‘research and consulting’.
Conclusion
These results will be utilized as basic data for future pediatric emergency nursing education initiatives and for establishing priorities of nursing policy to improve health care for children admitted to the emergency department.

Purpose
The purpose of this study was to investigate the influence of child-care efficacy, job efficacy, and health promoting lifestyle on the job satisfaction of in-home child care helpers who are called Idolbomi.
Methods
Participants for this survey were 153 in-home child care helpers who worked in Seoul. Data were collected from May 23 to October 17, 2014 using self-report structured questionnaires. Data were analyzed using IBM/SPSS 18.0 program.
Results
There were significantly positive correlations between child-care efficacy, job efficacy, health promoting lifestyle, and job satisfaction. Stepwise multiple regression analysis revealed that the predictors of job satisfaction were spiritual growth, education level, job efficacy, effect of income.
Conclusion
Therefore, we need to develop strategies to enhance the job efficacy and health promoting lifestyle of in-home child care helpers to improve their job satisfaction.

PURPOSE
This study was conducted to identify current health care management and barriers in health care management according to ecological systems, and to develop an ecological model for enhancing health care in child care centers.
METHODS
Focus group interviews were conducted with directors and teachers of child care centers, and with parents of children enrolled in child care. Data were analyzed by the latent content analysis method.
RESULTS
Twelve categories of health care management were identified. Barriers to child health care included knowledge deficit and lack of competence in health care by teachers, lack of useful health care manuals, non-existence of professional child health care personnel in child care centers, lack of mutual information sharing and disagreement on child health conditions between child care personnel and parents, lack of specific health related child care inspection criteria and time flexible child care centers with a lack of policy on collaboration with health care facilities. The ecological model developed included specific strategies to improve health care management in child care.
CONCLUSION
The proposed ecological model to improve child health care management should be useful to plan future health care program considering both the immediate and indirect social environment surrounding children in child care.

PURPOSE
To investigate the attitude, knowledge, and practice of safety and related factors of teachers in child care centers.
METHODS
The total sample consisted of 116 teachers from child care centers in Kyunggi Province. A questionnaire and checklist were used to collect data, which was analyzed with SPSS 11.5 Win program.
RESULTS
Most of the teachers were anxious about child accidents, and thought of safety as a primary concern. The mean score for knowledge on safety was 10.23, and the rate of correct answers ranged from 34.8% to 98.3%. The mean score on practice of safety was 42.01. Teachers with higher education showed significantly higher scores in safety efficacy. Teacher's knowledge on safety was significantly different depending on the experience of safety education, but this did not apply to safety practice. Safety practice was significantly related to confidence in safety performance, health beliefs on safety, stress, and social support, but not related to knowledge of safety.
CONCLUSION
Psychosocial factors were found to be important in safety practice. Related factors found in this study should be considered when providing safety programs for child care centers.

The Metaanalysis of Trends and Contents of Child Nursing Intervention Research.

The purpose of this study was to identify the trends and contents of intervention towards children using meta analysis, to support the basis for using in the field and research method about nursing intervention. We used 27 materials which was reported from 1970 to August, 1999 : dissertation study and Korean Nurses' Academic society Journals, the Journal of Korean Academic society of Adult Nursing, The Korea Journal of Maternal and Child Health Nursing. The types of intervention we used came from 3 different researchers. Snyder showed cognitive, movement, social sensory intervention. McCloskey & Bulechek categorized as the following : self-care assistance, acute care management, life-style alteration, health promotion, life support intervention, Craft & Denehy classified psychosocial intervention and biophysiological intervention. Some findings are summarized as follow : Out of the 27 researches sensory intervention had the most in there thesis, recently cognitive intervention research has a tendency to increase. 18 researches has acute care management in there theses, and health promotion was found the least. Out of the 27 thesis 15 thesis was classified as biophysiological intervention and 12 had psychosocial. 27 thesis had 11 types of interventions which originally was categorized by Snyder, therefore sensory intervention thesis had the most. 11 types of intervention which originally was classified by McClosky & Bulechek, teaching and information had the most out of acute care management. Out of 27 thesis, 14 had dealt with newborns, especially newborns with sensory intervention. Therefore school age and above had cognitive intervention which was used for teaching and information. Infants, preschool, schoolage children received acute care management the most, health promotion intervention was used towards adolescences. Depending on the characteristics of dependent variables, it was analysed using meta however 17 thesis are possible except primary experimental research. Mean effect size comparison by Snyder classification, cognitive intervention was the largest mean(1.51), sensory intervention was larger(0.71) also, movement intervention was in the middle(0.56) as shown. Comparison done by McClosky & Bulechek, the intervention leading to life style alteration was the largest mean(1.97), teaching was used the most. Comparison by Craft & Denehy classification, psychosocial intervention was larger(1.15) than biophysiological intervention (0.67). The result of nursing intervention through age classification, the largest weighted mean effect size in the research was towards infants and neonates. The research which was focused on nursing intervention, has important meaning in nursing practice and knowledge development. When we know that children`s nursing intervention is necessary and overcome our biased view, efficiency of children's nursing intervention are increased and professionalized. Therefore results will be important basic data to guide a development of child nursing intervention & classification.

A Study on Safety Knowledge, Health Beliefs and Self-efficacy of Child Care Teachers.

PURPOSE
The purpose of this study was to examine the relationships between safety knowledge, health beliefs and self-efficacy in child care teachers.
METHOD
The participants were 394 teachers from child care centers in Seoul city. Data collection was done using a questionnaire that included a Safety Knowledge Test, Health Belief Scale and Self-efficacy Scale. Descriptive statistics, t-test, ANOVA, and Pearson correlation coefficient were used to analyze the data. The SPSS 11.0 Win program was used for analysis.
RESULTS
The mean was 12.11 for safety knowledge, 38.31 for health belief, and 91.07 for self-efficacy. There were statistically significant differences in the degree of safety knowledge according to safety education, in the level of health beliefs according to length of service as a teacher and in the degree of self-efficacy according to age, and length of service as a teacher. There were statistically significant positive correlations between safety knowledge and health belief, and between safety knowledge and self-efficacy of child care teachers.
CONCLUSION
According to the results, programs to improve safety knowledge, health beliefs and self-efficacy of child care teachers should be developed and used to decrease the proportion of accidents involving children in Korea.
